Summary of Changes from Previous Version

Minor amendments to update links to declaration forms. Some responsibilities have been re-assigned to the Conflict of Interest Manager, rather than the discloser. Additional risk factors for consideration have been added to Clause 36, and the attachments for Risk Assessment Considerations and Conflict of Interest Management Options have been updated.
