This is the first version of this document to be published in the new UON Policy Library. For all intents and purposes, this version is reflective of the version that was approved, published and in effect at the time the new Policy Library was launched. In migrating the document to the new Policy Library it was subjected to reformatting, including the application of a new numbering system. The following information details the history of changes to the document prior to its publication in the new UON Policy Library: 12 March 2015 - Minor administrative edits to clause 5.3.2 to improve clarity of wording. 15 December 2014 - Minor administrative edit to replace reference to the Audit and Risk Management Committee with the Audit subcommittee. 24 October 2014 - Minor administrative edits made to update position titles and remove reference to rescinded policies. 14 August 2013 - The Secretariat - Appendices 2 and 3 edited due to amendments made to the NSW Oaths Act 1900, affecting statutory declarations and affidavits, effective from 30 April 2012. 16 November 2011 - Added Related Link to Conflict of Interest Guideline for Research Higher Degree Supervisors 000950 approved 18 May 2011. 23 August 2011 - Administrative amendments due to implementation of Student Misconduct Rule which replaced Student Discipline Rules effective 25 July 2011. Minor amendment 26 July 2011 - Clause 5.5 amended to reflect the need for staff to complete a declaration form until the information is included in HRonline. Minor changes to format of Declarations at Appendix 2 and 3 based on feedback received, Governance and Policy, 13 July 2011. Approved by Council - 18 March 2011.
